Be prepared for your notary visit:

Before visiting our office, please call (515) 244-9444 to make sure a notary is available and/or to make an appointment.
Please bring a valid, government issued photo ID with you.
Complete all original documents (no photocopies) EXCEPT for the signature and date fields. You MUST sign the document in front of the notary.
Bring your own witnesses (who are NOT related to you) if the document requires witnesses in addition to the notary.
Notary fees: $10.00 first page, $2.50 each additional page.
